National Benefit Funds The IATSE National Benefit Funds has launched its first Web site. Some of its features consist of downloading forms for claims, dependent and beneficiary designations, and reimbursements, Trust Acceptance Agreements and Documents and reporting forms. It also provides the Summary Plan Description Booklet for all the Funds; links to various carriers for health information and, members will be able to make payments by credit card for the Quarterly Health and Welfare Plan C CAPP Co-payments.

Caremark - Effective April 1, 2007, Caremark is IATSE's new prescription benefits manager, replacing Express Scripts. "Please note that the transition in prescription benefits managers will not require you to obtain a new prescription from your doctor sooner than would otherwise be necessary on the majority of medications."

Waiving Medical Coverage From Spring 2008 newsletter , page 2 "Now Hear This—Especially if You Have Other Coverage If you waive coverage, you must follow certain procedures to maintain your “other coverage” status, and we describe those procedures in these pages. In the Winter issue, for example, we reported that, if you have other coverage, you must provide valid proof of that coverage each year during annual enrollment. We also reported that if you have other coverage, a $150 administrative fee is deducted from your CAPP account each year.

Waiving Coverage? Keep Reading… Another reason for participants with other coverage to read our Health & Welfare Fund communications is that you’re still eligible to participate in the Medical Reimbursement Program. In fact, this Program should be of special interest to participants who waive coverage under Plan C. That’s because, under the Program, you can use any contributions in your CAPP account to reduce or eliminate premiums you pay to another plan and/or to pay for any number of qualified medical expenses that are not reimbursed by your other coverage."
